New short-term pathways video
The Department of Health, Disability, and Ageing has created a video explaining how Support at Home’s short-term aged care options work and when they might be right for you.

Support at Home is an Australian Government aged care program. The program helps older people live at home for longer. Along with ongoing services for regular care, it offers three short-term pathways for extra support if your needs change.
Restorative Care Pathway
The Restorative Care Pathway helps you to regain strength and confidence after illness or injury by delaying the need for long-term care. It provides up to 16 weeks of coordinated services, including allied health and nursing support.
End-of-Life Pathway
The End-of-life Pathway supports you if you have been diagnosed with three months or less to live and wish to remain at home. It works alongside state or territory palliative care and offers 12 weeks of services, which may include:
• Personal care
• Help around the house
• General nursing.
Assistive Technology and Home Modifications scheme
This pathway supports your independence by providing products and equipment that make your home safer and easier to get around.
Depending on your assessed needs, it provides access to funding for equipment and/or home changes for a period of 12 months.
